A Bradford firm has been ordered to pay £140,000 in fines and costs after they admitted safety failures regarding an unsecured racking system which crushed a 61 year old worker to death.

The reason for this incident, as stated in the court hearing, was that the company had no system in place to safely move or inspect racking, there were no load notices indicating maximum load capacities and the staff had limited training in the warehouse.
